Personally Identifiable Information (PII) refers to information that can be used to uniquely identify, contact, or locate a single person or information that can be used with other sources to uniquely identify a single individual. This includes information such as your name, email address, location, pain type, medication, interests and any other similar personal information that you may submit through this site. Goalistics does not sell or share email addresses with outside individuals or companies. To ensure that your PII is not inadvertently disclosed, we also ask that you keep your user name and password private and that you do not enter any information into the Goalistics system that could be used to identify you (e.g., your name, any part of your address, phone number, social security number, etc.)
Some of your data may be used for research purposes and/or amalgamated with other data and used for various studies and publications. If these data are used for publications, they will not contain any PII that would identify you. These data may also be used to improve the content of the site to help others manage their pain better. Goalistics administrators have full, password protected access to your PII as well as all other Electronic Protected Health Information (EPHI). Administrators and employees of Goalistics are required by law to keep any PII or EPHI confidential. By using this site, you agree to these uses of your PII and EPHI. You also agree that we may contact you with information about the Program. If you have any questions or concerns, please see the Contact Us section of the site.
